If this is your first visit to the Center, there are a few things that you will need to do as a first time patient.

1. You will need to fill out first time patient forms.  If you download these forms, fill them out and bring them with you it will shorten your wait.  Visit our Forms Page to download your forms now.
2. You bring copies of your medical records including cultures, hospital records, clinic records, copies or reports and diagnostics related to your issue. Or, you can arrange to have the information sent to our office prior to your visit. Doing this will greatly improve the quality of your visit and may save you the necessity of an additional follow-up visit.
3. You should bring ALL of your medications, current and recent, with you in a large ziploc plastic bag EVERY visit.
4. You should bring your insurance card and method of payment.  We do accept most major credit cards and will work with you and your insurance provider to determine how to best participate with your insurance provider.  Please note that many travel services are not covered by insurance, but there are some that are eligible.
